I haven't ran Quake,
but if it is sitting in the Program Files folder,
then yes. As Windows 7 doesn't allow you or the programs to write in
some areas and that is one of them. The way around this problem (if that
is what it is) is to install the program in another folder (i.e. Games,
etc) which Windows 7 doesn't restrict excess to at least with
administrator privileges.

I copied my Quake III Arena directory right to the C:\ drive and viola, it
works. I started my game and went into setup then clicked on the
defaults option and lo and behold my empty config file filled up with all
the settings. You'll still have to do any custom config work manually.
Problem solved
